<p> <span style="color: rgb(51, 51, 51); font-family: Roboto, Arial; font-size: 14px; white-space-collapse: preserve-breaks;">Tổ chức đ&aacute;nh gi&aacute; sự ph&ugrave; hợp khi c&oacute; nhu cầu đăng k&yacute; tham gia hoạt động thử nghiệm lập 01 bộ hồ sơ, gửi về Ủy ban nh&acirc;n d&acirc;n tỉnh, th&agrave;nh phố trực thuộc trung ương (sau đ&acirc;y viết tắt l&agrave; cơ quan tiếp nhận hồ sơ) a) Trường hợp hồ sơ kh&ocirc;ng đầy đủ theo quy định, trong thời hạn 05 ng&agrave;y l&agrave;m việc, kể từ ng&agrave;y nhận được hồ sơ, cơ quan tiếp nhận hồ sơ th&ocirc;ng b&aacute;o bằng văn bản y&ecirc;u cầu tổ chức thử nghiệm sửa đổi, bổ sung; b) Trường hợp hồ sơ đầy đủ v&agrave; hợp lệ, trong thời hạn 10 ng&agrave;y l&agrave;m việc, kể từ khi nhận được hồ sơ, Ủy ban nh&acirc;n d&acirc;n tỉnh/th&agrave;nh phố chịu tr&aacute;ch nhiệm cấp Giấy chứng nhận cho tổ chức thử nghiệm theo Mẫu số 10 tại Phụ lục ban h&agrave;nh k&egrave;m theo Nghị định số 154/2018/NĐ-CP ng&agrave;y 09 th&aacute;ng 11 năm 2-18 của Ch&iacute;nh phủ; c) Thời hạn hiệu lực của Giấy chứng nhận kh&ocirc;ng qu&aacute; 05 năm kể từ ng&agrave;y cấp.</span></p>

T&agrave;i liệu chứng minh năng lực hoạt động thử nghiệm đ&aacute;p ứng c&aacute;c y&ecirc;u cầu quy định tại khoản 2 Điều 5 Nghị định số 107/2016/NĐ-CP, cụ thể như sau: + Trường hợp tổ chức thử nghiệm đ&atilde; được tổ chức c&ocirc;ng nhận quy định tại Điều 21 Nghị định số 107/2016/NĐ-CP hoặc tổ chức c&ocirc;ng nhận nước ngo&agrave;i quy định tại Điều 25 Nghị định số 107/2016/NĐ-CP c&ocirc;ng nhận hoạt động thử nghiệm, tổ chức thử nghiệm nộp bản sao Chứng chỉ c&ocirc;ng nhận k&egrave;m theo phạm vi được c&ocirc;ng nhận. + Trường hợp tổ chức thử nghiệm chưa được c&ocirc;ng nhận, tổ chức thử nghiệm nộp c&aacute;c t&agrave;i liệu, quy tr&igrave;nh thử nghiệm v&agrave; c&aacute;c t&agrave;i liệu kh&aacute;c li&ecirc;n quan để chứng minh năng lực hoạt động ph&ugrave; hợp với c&aacute;c y&ecirc;u cầu của ti&ecirc;u chuẩn tương ứng quy định tại khoản 2 Điều 5 Nghị định số 107/2016/NĐ-CP</td>

1. L&agrave; tổ chức được th&agrave;nh lập theo quy định của ph&aacute;p luật.</div>

C&oacute; hệ thống quản l&yacute; v&agrave; năng lực hoạt động đ&aacute;p ứng c&aacute;c y&ecirc;u cầu quy định trong ti&ecirc;u chuẩn quốc gia TCVN ISO/IEC 17025:2007 hoặc ti&ecirc;u chuẩn quốc tế ISO/IEC 17025:2005 hoặc ti&ecirc;u chuẩn quốc gia, ti&ecirc;u chuẩn quốc tế đối với thử nghiệm chuy&ecirc;n ng&agrave;nh.</div>

C&oacute; &iacute;t nhất 04 thử nghiệm vi&ecirc;n ch&iacute;nh thức của tổ chức (vi&ecirc;n chức hoặc lao động k&yacute; hợp đồng c&oacute; thời hạn từ 12 th&aacute;ng trở l&ecirc;n hoặc lao động k&yacute; hợp đồng kh&ocirc;ng x&aacute;c định thời hạn), được đ&agrave;o tạo về ti&ecirc;u chuẩn quốc gia TCVN ISO/IEC 17025:2007 hoặc ti&ecirc;u chuẩn quốc tế ISO/IEC 17025:2005 hoặc ti&ecirc;u chuẩn quốc gia, ti&ecirc;u chuẩn quốc tế đối với thử nghiệm chuy&ecirc;n ng&agrave;nh.</div>

Trường hợp bổ sung lĩnh vực hoạt động thử nghiệm, phải c&oacute; &iacute;t nhất 02 thử nghiệm vi&ecirc;n ch&iacute;nh thức của tổ chức (vi&ecirc;n chức hoặc lao động k&yacute; hợp đồng c&oacute; thời hạn từ 12 th&aacute;ng trở l&ecirc;n hoặc lao động k&yacute; hợp đồng kh&ocirc;ng x&aacute;c định thời hạn) tương ứng với lĩnh vực thử nghiệm đăng k&yacute; bổ sung, được đ&agrave;o tạo về ti&ecirc;u chuẩn quốc gia TCVN ISO/IEC 17025:2007 hoặc ti&ecirc;u chuẩn quốc tế ISO/IEC 17025:2005 hoặc ti&ecirc;u chuẩn quốc gia, ti&ecirc;u chuẩn quốc tế đối với thử nghiệm chuy&ecirc;n ng&agrave;nh.</div>
